Return Materials Authorization

Should you need to return a product for service, please follow the following instructions:

  1. Fill in all the appropriate blanks on the RMA template. Click here for copy of form.
  2. Email the RMA request form to rma.cti(at)cornet.com who will issue an RMA number and return the form to you.  We will issue an RMA number for merchandise under warranty or service agreement. 
  3. Include a copy of the returned RMA form with your return shipment.
  4. Write the RMA number boldly on the side of the box.
  5. Ship via pre-paid freight to:  Cornet Technology, Inc.; 6800 Versar Center, Springfield, VA 22151

Repaired equipment will be returned prepaid via the best carrier.  Should you require priority freight, please provide a carrier account number.

How to fill in the form:

  1. Client: your company's name
  2. Date:  date of the request
  3. Bill to:  billing address if product is out of warranty or not under maintenance contract
  4. Ship to:  address for the repaired product is to be returned
  5. Contact Name:  name of person requesting the RMA
  6. Phone#:  number where you can be reached if we have questions
  7. Contract/PO#:  number issued for purchase of maintenance agreement 
  8. Part Number:  this number typically starts with the letter C
  9. Item Description:  name of part being returned, e.g. controller board
  10. Serial Number:  serial number of the part being returned.  Serial numbers for boards are found on silver labels; those for chassis are found on white labels either on the back or side of the chassis.
  11. Description of problem:  be as precise as possible as this will assist in problem diagnosis
